HIPAA Notice of Privacy Practices - Counseling on Call

Counseling on Call is committed to protecting your health information across Maryland, Virginia, Florida, Oregon, Washington, Arizona, and Iowa. This Notice of Privacy Practices describes how we may use and disclose your Protected Health Information (PHI) and your rights regarding that information. Please review it carefully.

WHO WE ARE

Counseling on Call provides secure telehealth therapy, couples counseling, family therapy, and medication management. We are a HIPAA-covered entity operating in multiple states. Our providers are licensed in MD, VA, FL, OR, WA, AZ, IA.

HOW WE USE AND DISCLOSE YOUR PHI:

1. For Treatment: We use your PHI to provide mental health treatment. This includes conducting initial assessments, creating treatment plans, conducting therapy sessions via our HIPAA-compliant telehealth platform, coordinating care with your primary care doctor or psychiatrist (with your written consent), and referrals to specialists.

2. For Payment: We use PHI to bill and collect payment. This includes verifying your insurance benefits with Blue Cross Blue Shield, CareFirst, Aetna, Cigna, United Healthcare, Optum, Anthem and other major payers, submitting claims to your insurance via our billing partner Headway, determining your copay, coinsurance, and deductible.

3. For Healthcare Operations: We may use PHI for quality improvement, reviewing competence of providers, training our 12+ licensed counselors, business management, and customer service.

4. Other Uses Required by Law: We may disclose PHI when required by federal/state law, for public health activities, to report abuse/neglect of child or vulnerable adult, for judicial proceedings with court order, for law enforcement purposes, to avert serious threat to health/safety.

YOUR RIGHTS REGARDING YOUR PHI:

You have the right to: Access your therapy records and billing records, request amendment if you believe information is incorrect, request confidential communications at alternative address, request restrictions on certain uses, receive paper copy of this notice, file complaint if you feel rights violated.

OUR TELEHEALTH PRIVACY MEASURES: All sessions use 256-bit encryption, HIPAA-compliant video, no session recording without explicit written consent, secure EHR system, staff trained annually on HIPAA.

RETENTION: We retain therapy records for minimum 7 years (10 years for minors) as per Maryland and other state laws.

CHANGES TO THIS NOTICE: We reserve right to change this notice. New notice will be posted on www.counselingoncall.net/hipaa-notice and effective for all PHI we maintain.
