Cancellation

If you are unable to attend an appointment, kindly provide 24 hours of notice. Please note, since we are unable to use this time for another patient, cancellations with less than 24 hours notice, and missed appointments will be charged 75 dollars. Insurance companies would not cover for missed appointment fees. You will be responsible for the payment.

Confidentiality

Clinical records are confidential. Information about your treatment will not be released without your written permission with few exceptions. We are required by law to report evidence of suicidal or homicidal intent, evidence of past or current child abuse, and evidence of past, or current elder abuse. Confidentiality can also be broken if the information could help save the patient's life in a life-threatening emergency. Information may also be shared with medical providers who are concurrently providing medical treatment for you to promote coordination of care and the benefits to you of shared knowledge.

Financial

Payment is expected at the time of your visit. We accept Cash, check, or credit card. Payment could be any deductible, co-insurance, co-payments, non-covered covered charges from your insurance company.
